-About Optilife Insurance and Payment Policies:

Optilife continues to work to increase the number of insurance providers we are in network with today. We have contracted with most of the top Tampa insurance companies to be part of their provider network. As a result, we are giving our patients the most affordable chiropractic health care possible.

Accepted Insurance Payment Process:

On or before your first visit, our staff will check your insurance benefits for chiropractic care and determine the following:

• if there is a deductible that must be met
• what your co-pay may be
• how many annual visits are allowed
• what your annual out-of-pocket max is set to

Give us your insurance information, and we will look up your benefits on our automated system.  Sometimes, we must call the insurance company directly for specific benefit information.  This may cause a slight delay in confirming your benefits.

Once benefits are confirmed, our billing system and dedicated billing specialist will automatically submit your treatment invoices to your insurance company for you.  You’ll be notified if there are any problems or delays in processing claims.  If insurance payments are not made, you will be responsible for unpaid charges at the contracted rate.  Therefore, we encourage you to check your insurance statements regularly to ensure proper payments are made.

You will be responsible for paying for treatments in full at the time of the visit if your deductible not been met.  Additionally, all co-pays must be paid with each visit.
